The Greek word tarsos first described a flat woven mat or basket, the kind used for drying fish along the Aegean coast. By the 5th century BCE, physicians extended it to the broad flat sole of the human foot, the surface that presses against the earth with every step. Aristotle used it in his biological writings to describe the padded undersides of animal feet. The image was always spatial: a flat expanse where weight meets ground.
Galen of Pergamon, writing in the 2nd century CE, inherited tarsos from earlier physicians and applied it with precision to the seven bones forming the posterior half of the foot. His Latin transliteration, tarsus, became the canonical term in every anatomy manual copied across medieval Europe. Students at Bologna and Paris memorized the tarsus as a unit before learning each bone's individual name. The Greek basket had become a Latin anatomical province.
The adjective tarsal crystallized in English during the 18th century as surgeons began writing in vernacular alongside Latin. William Cheselden's Osteographia of 1733 used it to systematically distinguish the tarsal bones from the metatarsals forward of them. The seven individual bones, calcaneus, talus, navicular, cuboid, and three cuneiforms, each earned their own specific name in addition to this collective one. No single bone owns the word tarsal; it belongs to the zone as a whole.
Long before modern orthopedics, soldiers and shoemakers understood the tarsal region by feel rather than name. Roman legions fitted sandals to support the tarsal arch, because a legionary whose ankle collapsed was one who could not march twenty miles. Today's podiatrists, orthopedic surgeons, and sports trainers use the same word for the same seven bones, unchanged since Galen organized them in the 2nd century.
